Before we can begin to process your reservation we need a completed intake form (one per group). The Team Intake Form can be found in the Form Packet below. Before arrival, please send or fax completed release and skill assessment forms for each member of your group. Youth under 18 should fill out the Youth Release form. All forms can be found in the Form Packet.

Each volunteer must fill out a release form and a medical form. The medical form should be kept by your group leader at all times. We do not need your med form–it’s for your group leader to use in case of an accident on site.
